How do I setup my account?

Setting up the account is an easy, one time process. You can setup your account as follow:

  1. Go to the account registration page.
  2. Fill out the Registration page with your clinical and office information. The e-mail address you submit will become your future login.
    Note: You may register more than one office for your account. When filling out each biopsy form, you will be able to easily select which offices you would like the pathology report sent to.
    Click on <Register>
  3. If errors appear in red at the top of the page, fill in additional requested information and re-click on <Register>.
  4. When you have successfully registered, you will be brought back to the main page. A confirmation e-mail will be sent to you with your registration information.

Registering an account will enable you to complete biopsy requisition forms online.

How do I modify/update my account?

You may change information in your registration profile by clicking on the link to <My Profile> (upper right corner) after login. In the “My Profile” page, you may edit and save any changes to your account information.

I have forgotten my password / I need a password

If you need to reset a password, click on <Forgot your password?> from the main entry page. You will need your e-mail you used to register your account. A link will be sent to your e-mail that allows you to set a new password.

Password policy

All users are responsible for taking the appropriate steps, as outlined below, to select and secure their account passwords.

In order to minimize the risks of unauthorized access and maintaining password confidentiality, user passwords must be non-obvious, hard-to-guess, and changed on a regular basis.

Passwords should be changed regularly, e.g. every 42 days;

  • Users are required to select a "strong" password:
    • the minimum acceptable password length is 7 characters.
    • the password should include both characters and numbers.
    • the password must not be constructed based on personal information (e.g. birthday, first/last name and telephone number).
    • the password must not be constructed by adding a number/character to the beginning/end of the word in rules above.

Inappropriate use of passwords includes, but is not limited to:

  • Revealing your password to anyone.
  • Writing your password down on any visible media or through e-mail.
  • Transferring an unencrypted password electronically.
  • Storing an unencrypted password in a file on any computer system (including portable devices).

How do I send the x-rays and/or clinical photos?

X-rays/radiographs and clinical photos or digital images of a specimen can be sent:

  1. Electronically, via e-mail to obs@dentistry.ubc.ca (preferred)

    OR

  2. By mail to:

Note: For digital radiographic images, we require that the digital file be sent to us since the oral radiologist will not comment on the hardcopy of the digital images.

How do I send the specimens and forms?

For most diagnoses, the sample should be put in a biopsy kit containing 10% buffered formalin, which can be obtained by phoning Surgipath (1-800-665-7425) or through another pharmacy. If a diagnosis of benign mucous membrane pemphigoid, lupus erythematous or pemphigus vulgaris etc., vesiculobullous conditions is suspected, half of the sample should be put into Zeus’ solution in addition to the specimen for formalin fixative. This special fixative can be obtained by phoning Vancouver General Hospital Immunohistochemistry Department (604-875-4111 ext. 63966).

During winter when the temperature is below zero, the sample should be fixed in buffered formalin at room temperature for several hours or overnight to prevent freezing artifact, or to be sent by a courier company with temperature regulation.

All specimens (along with printed and signed requisition form) must be sent by mail or courier to:

For samples sent by mail, it often takes two weeks to receive the specimen. It is recommended that you make the follow-up appointment with the patient at least 3 weeks following biopsy as we cannot guarantee a faster response.

For faster turnaround time, samples should be sent by overnight courier. If urgent, mark RUSH on the requisition form and submit by overnight courier. In these cases, reports can be faxed 24 hours after receiving the specimen.

What is the expected report turnaround time?

For samples sent by mail, it often takes 2 weeks to receive. It is recommended to make the follow-up appointment 3 weeks or later as we cannot regulate the length of the delivery.

For faster turnaround time, samples should be sent by overnight courier.

On rush specimens (please mark RUSH on the requisition form), reports can be faxed 24 hours after receiving the specimen.
